Managing frequent attenders to hospital

Many chronicly ill patients attend hospital many times over the year. During the covid crisis they have been absent from hospital. When restrictions are lifted we may find a flood of patients attending A&E and then filling up wards. To avoid this why don't we start nursing these patients at home now, there by avoiding overcrowding in the hospital. Many nurse are surplus at moment especially bank workers why not use them.
