There should be focus group set up by the community AHP lead to get other together all the Allied Health Professionals (Physios, OT, social worker, Mental health) to come up a framework / rehab need assessement tool to determine and prioritise and action interventions to help vulnerable people missing out physical and mental health rehabilitation. Could set up remote exercise groups - video calls.

To ensure and action appropriate physical and mental health rehabilitation.
